Prize Redemption

In order to claim and receive your prize, you need to provide us with the required eligibility information listed below. Best part is, you only have to do this once (as long as you don't move from your current address).

Eligibility consists of proof of identity, proof of residency, and proof of age. All three (3) of these requirements must be met in order to claim your prize. The following forms of identification are acceptable as proof of identity and age:

• Valid US Driver's License
• Valid US State Identification Card
• Valid US Military Identification Card
• Valid Canadian Driver's License
• Valid Canadian Province Identification Card
• Valid Passport

You can submit a copy of the above proof, along with your full name, AND your ClubWPT Username, to us in any one (1) of the following ways:

If you have won $600 or more in any given calendar year, Federal Tax Regulations requires that you complete and return form W9 and that we issue you a form 1099 at the end of each calendar year.

If you prefer, you can fill this out at any time to avoid future delay. You will only be issued a form 1099 if you met or exceed the Federal Tax requirement. Please note that you are NOT required to submit a W-9 form unless your cumulative annual winnings are $600 or more.
